National Eye Institute Visual Functioning Questionnaire - 25

Overview

The VFQ-25 is a reliable and valid 25-item version of the 51-item National Eye Institute Visual Function Questionnaire (NEI-VFQ).

The following is a survey with statements about problems which involve your vision or feelings that you have about your vision condition. After each question please choose the response that best describes your situation.

The survey will take approximately 10 minutes to complete, however there is no time limit and please take as much time as you need. In order for this survey to improve our knowledge about floater-related eye-problems and how they affect your quality of life, your answers must be as accurate as possible.

Please answer all the questions as if you were wearing your prescription glasses or contact lenses (if required).

If you have any questions, please feel free to contact us and we will try to help.
